a. I am not experiencing: a high temperature, new or persistent cough, a loss or change to your sense of smell or taste

b. I have not had a Covid-19 test that proved positive with the last 14 days

c. If I travelled abroad to a listed restricted country, I self-isolated as defined by governmental policy

d. I am not currently self-isolating
